Your GP practice is starting a new program to help you manage your own Blood Pressure. The care co-ordinator team are contacting patients and asking them to measure their blood pressure using a home monitoring machine, twice a day for four days in a row. After you have completed the seven days of readings and sent the results back, the team will contact you with your feedback.
If you know you have high Blood pressure or have recently had a high reading and want to take part in home blood pressure readings, please see the BP Patient Pack and take part or for more information on home blood pressure please contact the care co-ordinators via email occg.schc_care_coordinators@nhs.net
