RBH has launched an online pregnancy notification form. Please encourage patients to use this form if they have recently found out they are pregnant and would like to book their first midwife appointment at the Royal Berkshire Hospital Maternity | Royal Berkshire NHS Foundation Trust
Patients must be registered with a local GP and living in the catchment area. Please inform patients they should not complete this form if they do not intend to give birth at RBH. |

The District Nursing team’s role is to assess the health needs of people who are housebound and plan a programme of care to meet those needs.
Employed by Oxford Health NHS Foundation Trust, working both independently and collaboratively with the Primary Health Care Team and based at Townlands Hospital. Liaising with statutory and voluntary agencies such as social services, specialist resources and associations, is also a very important part of their work.
A patient is deemed to be housebound when they are unable to leave their home environment through a physical or psychological illness. A patient is not considered housebound if they are able to leave their home with minimal assistance. For example unassisted/assisted visits to the doctor, dentist, hairdresser, supermarket, social events. Patients requiring transport can contact the FISH voluntary agency or taxi services. FISH also runs a weekly bus service to and from Binfield Heath.
As well as providing professional support, skilled nursing care, health education and advice to patients, relatives and carers, they also teach undergraduate and postgraduate nursing students, health care assistants and medical students.

Please note that the midwife is not at the health centre at the current time.
Midwife booking self-referral form
Royal Berkshire Hospital has now gone live with a self-referral form for pregnant women to request their first midwife appointment in addition to still being able to book by phone on 0118 322 8964 via option 1. Please encourage use of this with your patients. The form can be translated into 10 languages, with any other added if the need arises.
