Access to your online medical record

You are now able to access your online medical record.

Access to your medical record includes the ability to prospectively view:

  • Your online medical record
  • Consultation notes
  • Diagnosis
  • External documentation
  • Repeat Prescriptions
  • Upcoming Appointments (with the ability to book certain appointments)
  • Past and future test results/ Immunisations

To submit an access request form, complete the form below

We aim to action all requests within 14 days, although often we manage this much sooner.  If there are any issues with this request, we will contact you directly.

PLEASE READ: Key considerations when accessing your medical record online

(Note: All other previous medical history will not be visible; only entries to your medical record on or after the date you are given access will be visible)

Should you require any additional access not listed above (past consultations, past problems), Please collect a further online access request form from the reception desk. You will need:

  • A proof of address dated within the last 3 months
  • A photographic form of ID

Processing for further online access may take up to 4 weeks.
