Access to your online medical record

You are now able to access your online medical record.

You should have basic access to your medication and appointments via the NHS App/NHS App website. 

However you may wish to have futher online access to your medical record, including the ability to prospectively view:

  • Your online medical record
  • Repeat Prescriptions
  • Upcoming Appointments (with the ability to book certain appointments)
  • Past and future test results/ Immunisations
  • Past and future Diagnosis
  • Consultation notes
  • External documentation

We aim to action all requests within 14 days, although often we manage this much sooner.  If there are any issues with this request, we will contact you directly.

Consent to online access to your medical records
By selecting 'Yes', you will have prospective access to your online medical record, making consultations, diagnosis, test results, paper records and more available to view. (Note: Past entries will not be visible)

 

(Note: All other previous medical history will not be visible; only entries to your medical record on or after the date you are given access will be visible)

Should you require past consultations added to your account on the NHS app, Please collect a further online access request form from the reception desk. You will need:

  • A proof of address dated within the last 3 months
  • A photographic form of ID

Processing for further online access may take up to 4 weeks.
