Summary Care Record – Ensuring your care needs are transferred between services

Summary Care Record

The Summary Care Record is an electronic record of your important patient information, created from GP medical records. It can be seen by authorised staff in other areas of the health and care system involved in your direct care.

The care record allow healthcare professionals to see:

  • current medication
  • allergies and details of any previous bad reactions to medicines
  • your name, address, date of birth and NHS number

Access to this information means that care in other settings (hospitals) is safer, reducing the risk of prescribing errors, and helps avoid delays to urgent care.

Your SCR is created automatically when registered with a GP practice in England. 98% of practices are currently using the system.

You can talk to your practice about opting out of including Additional Information about long term conditions, care preferences or specific communication needs.
