Requesting Proxy Access for GP Online Services – Adults 16 and over

1) The Proxy user/user applying for access must First:

Register for online services; you will need to verify your identity by either:

• Register via the NHS App or https://www.nhsapp.service.nhs.uk/login
• Complete this form and then visit the practice, bringing with you two forms of identification.
  • One of these items should include your photograph.
  • We will then issue you a username and password.

Once you are registered you will be able to use the service for yourself to:

• Order your repeat prescriptions
• Make/cancel an appointment
• Change your contact details
• Review your medications and known allergies
• Review your consultation record

We aim to action all requests within 14 days, although often we manage this much sooner.  If there are any issues with this request, we will contact you directly.
