Summary Care Record (SCR) update: as a result of Covid 19 additional information will be automatically added to patients’ summary care records (SCRs). This additional information includes details of long term conditions, relevant medical history and any personal or healthcare preferences. Providing this information on patients’ summary care records will help healthcare staff outside this practice to provide the best possible care to you, should it be needed at this critical time. This update to your summary care record will be reviewed when the coronavirus outbreak is under control.
