The Care Co-ordinator team’s role is to work alongside the GPs and nurses to ensure the best and most up to date care is provided to all our patients.
We are also here to work with all our patients to support you to make informed decisions about what health prevention steps you may wish to take.
We are currently assisting patients with:
- Blood Pressure Monitoring
- Cholesterol Monitoring
- Diabetes checks
- Asthma review
- NHS Health Checks
- Co-ordinating the Covid vaccine campaign.

Social Prescriber Team
Working alongside your GP and other practice clinicians, your social prescriber or community link worker, can help connect you to all the services, support, activities and opportunities that exist locally.
If you are aged 18+ and need help at home, want to discover more about local activities or find out about benefits and support services that exist – we can help you.
We will work together with you to support you to live as independently as possible.
