Self-Certification - Sick Note
If this is your first sick note for this illness and you have been suffering with it for 7 days or less then you don't need to see a doctor.
- You can complete a self certification form yourself
- Some employers do require a doctor's sick note regardless
- Many employers however have their own self-certification forms, but if they don't you can print and fill out the form below.

We stopped ear syringing at the end of March for all patients except those who the hospital's Ear, Nose & Throat department have requested it for or pre-referral to the Audiology (hearing) clinic. Patients can contact the following clinics to arrange ear syringing privately:

Turning Point offers support to individuals in Oxfordshire from full-time, dedicated recovery hubs.
The Turning Point team know that drugs and alcohol can affect many areas of your life and successful treatment start with being there. The team get to know you as a person, to understand what it is that you need to help you turn your life around. They work closely with other organisations in the county to ensure that help is there when you need it.
