Announcement - Dr Andrew Burnett will retire at the end of March 2017 - please see the attached letter. Dr Burnett retirementIf you are  currently a patient of Dr Burnett, from 1st April your usual GP will be Dr Claire Webster if your surname is A-J and Dr Nick Smith if your surname is K-Z. You can of course see any of our GPs, subject to availability. 

Some of you would have received a text message announcement.  Please contact Reception if you would like to receive text message announcements but we don't have your mobile details on record.


West Berkshire Breast Screening service will have a unit in the health centre car park for 6 weeks from w/c 6th March. The unit will take up several car park spaces in our car park. We will be closed on Friday 14th April and Monday 17th April inclusive, for the Easter period.  We are open as normal aside from these dates. Please ensure you have adequate repeat medication for the Easter period - we recommend that you submit any repeat prescriptions by 6.00 Friday 7th April for collection by Thursday 13th April.

 

Sonning Common Health Centre is a surgery in South Oxfordshire, a few miles from Reading.  We accept new patients from a large area, ranging from Nettlebed in the north, North Caversham in the south, Woodcote to the east and Henley to the west.  Click here to view a map showing our practice area.

Registration

Registration with the Health Centre is easy and you now have an online alternative. You can either come to reception bringing your valid ID.   We will ask you to fill in a questionnaire about your current health, and also give you a form to complete regarding Summary Care Record.  Please note that we require both of these forms to complete your registration.  You can also fill in the pre-registration form online (see below) and take it from there. We offer all new patients a health check. You will register with one doctor for routine consultations.  However, in the case of sudden illness or emergency, any of our doctors will always be happy to see you if your own doctor is not available. Children under five are additionally registered with Dr Ralph Drury, Dr Kim Emerson or Dr Ellen for developmental checks.

Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form.  When you have completed all of the details, click on the "Send" button to mail your form to us.  When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.  Please note that completion and submission of the form does not automatically entitle you to join the practice.

When you register you will also be asked to fill out a medical questionnaire and complete the summary care record forms. We need the medical questionnaire because it can take a considerable time (sometimes up to 6 months) for us to receive your medical records. There is an online version of this form too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Binfield Heath and Shiplake Residents

Patients from Binfield Heath and Shiplake may book transport to the surgery via FISH, a voluntary agency. Ring (0118) 972 3986 allowing an absolute minimum of 48 hours notice.  A car will always be available to help you, if booked in advance.

Messages and requests for medicines (which can be delivered to your home by arrangement) may be left at Binfield Heath Post Office, by kind permission of the proprietor.
